Subject: Primary Care Networks launch in Somerset

Thirteen Primary Care Networks started working in Somerset on 01 July and each is led by a GP or a nurse. While patients won’t see any sudden changes, they will, over time, see a wider range of clinical services available in their local GP surgery along with a greater variety of health and care practitioners.

GP practices in Somerset have been working in collaborative networks for several years, with a particular focus on patients with multiple health conditions or who are physically frail. The 2019 national GP contract settlement formalises this, with GP practices forming themselves into Primary Care Networks which serve populations of between 30,000 and 50,000 patients.

  • Offer groups that build the resilience and independence skills of Young Carers in Somerset;
  • Offer groups that provide opportunities for Young Carers to learn new skills and try new things;
  • Provide safe, accessible and welcoming spaces for Young Carers to meet their specific needs;
  • Ensure 4 sessions are offered per calendar month, made available from August 2019.

All decisions to award will be made by local area panels which will include Young Carers, with decisions being published week beginning 15 July 2019.
